Terms and Conditions for Use of MyChart

Please read these MyChart Terms and Conditions of Use carefully. MyChart is a convenience offered to our patients. Our goal is to have MyChart available 24 hours a day and seven days a week. However, we cannot and do not guarantee this availability.

MyChart Services

MyChart will provide you the ability to: view your medications; view your health conditions; view your allergies; update your demographic information and much more. You should never change or stop your provider's current treatment plan based on information provided in MyChart without first discussing it with him or her.

Emergency/Urgent Care

MyChart should not be used to communicate with your provider or his/her medical office regarding an emergency or urgent situation. IN AN EMERGENCY, YOU SHOULD CALL 911 OR GO TO THE NEAREST EMERGENCY DEPARTMENT.


When you register for MyChart, you will create a User ID, password and hint questions. It is your responsibility to protect this information in order to maintain the security of your MyChart account. We will never share this information with third parties. MyChart uses secure technologies to protect your information. In order to securely access your patient data within the MyChart, you will need to be signed in to your account. You are responsible if you intentionally or unintentionally share your MyChart user ID, password, or medical information contained in MyChart with others. MyChart is compliant with pertinent federal and state privacy/security laws and regulations.

Responses to Your Requests

We will strive to respond to any questions or requests within two business days however we cannot guarantee this timeframe. If you do not receive a response within two business days, please call your provider office for an update. Requests for paper copies of medical records may take longer to process.

E-Mail Notifications

When we have a MyChart notification to send you, a message will be delivered to the e-mail address that you have provided us. This message will not contain any health information, but will let you know that updated information is available when you log in to your MyChart account. The messages delivered to and from your MyChart account may be added to and stored as part of your electronic health record.


These Terms and Conditions may be modified at any time without prior notification to you. You will be notified of any revision via email.
